"Updated daily" is a claim. This page is the evidence.

Payor rules move, and a reference product that pretends otherwise doesn't deserve your trust. Here's how the pipeline actually works, what we promise, what we don't — and a public log of every correction we've applied, including the ones where we were wrong.

How the data stays current

Daily source checks

Agents poll every registered payor policy source daily and record the check timestamp. That timestamp is what you see on each payor page — 'Source checked 3h ago' means exactly that.

AI verification, human judgment

When a source changes, an AI pass extracts what changed and drafts a structured diff against the current record. Material changes get logged with the source document linked and an effective date — not the date we noticed.

Confidence tiers, shown honestly

Every field carries a confidence tier. 'Needs Review' means exactly that — we show it rather than hide it, so you know which fields to verify before relying on them.

Corrections, in public

Anyone can submit a correction with source evidence. Reviewed corrections get applied to the dataset and logged below — including the ones where we were wrong.

What we promise — and what we don't

Every rule links its source document — you can spot-check us in two minutes.
Change alerts fire the day our agents detect the change, with the source linked.
Applied corrections are logged publicly on this page. We don't quietly fix mistakes.
Confidence tiers are shown per field. 'Needs Review' fields say so.
We do not promise the database is a substitute for payor confirmation on high-stakes claims. It makes verification fast; it doesn't remove it.
We do not backdate or hide detection misses. If we caught a change late, the log shows when we caught it.

The most recent policy changes we logged, with the gap between each policy's effective date and the day our pipeline caught it. Late catches are labeled as late — hiding them would defeat the purpose of this page.

The correction log

Every reviewed-and-applied correction to the dataset, newest first. Submitted by users with source evidence, verified before applying. An empty month here is good news; a hidden mistake never is.
